Application & Enrollment Information

American Medical School Applicants

The application package must include:

• An application completed by the student and the Dean of the home school. The School seal must be affixed prior to submission of the application. Please submit your CV and transcript along with your ob-gyn elective applications.
• A NYU Visiting Student Medical Clearance Form including proof of Rubella immunity and screening for TB.
• A photocopy of the student's personal insurance card.
• Official malpractice insurance coverage documentation from the home school.

All four should be sent together and all four must be received before the applicant is enrolled. Applicants should also select alternate dates and/or electives. A new application must be sent for each alternate elective. To access application forms, please use the following link:

Enrollment Information

Students who are approved:
Once a confirmation letter is received, contact the preceptor to confirm your starting date, arrange meetings and discuss further details. On the first day, a one time $100.00 registration fee payable at Registration (check or money order only) is due. There is a $5.00 service fee for ID badges. All visiting students must report at 9:00am to the:
